Family physicians and their clinic team members are invited to participate in a new MyQ offering focused on improving care coordination with referral partners. Participants will be supported in choosing a referral partner, developing ways to send and receive information effectively, and clarifying roles and responsibilities for physicians, care teams, and patients.
The new virtual MyQ offering, Navigating Care Coordination series, engages teams to develop new processes to reduce care gaps and delays, while enhancing communication and information flow for patients and the team.
