Group Governance Agreement

The Group Governance Agreement Template has been customized for physicians practicing in blended capitation groups in Newfoundland and Labrador. Along with the Group Governance Checklist, it is intended to help you develop a group governance agreement. A group governance agreement is a formal written document that addresses the general operations of a group practice and is strongly recommended to document group intentions, and to mitigate potential problems in the future.

How you use the template will depend on your practice situation. The FPRP recommends meeting with your group to discuss the topics outlined in the Group Governance Checklist to set your intentions as a group before seeking legal or accounting advice, and then to leverage the Group Governance Agreement Template as you formalize your arrangements. The template represents one option for cost-sharing in a group practice, but it is not the only way you may choose to arrange your group practice. It is not meant to be prescriptive: some groups may choose to use the template directly, while others may want to use it as a starting point for customization. Or, if a group already has a group agreement in place, the template may be used to help inform adjustments to address new aspects of group practice that come with blended capitation. The template is not intended as accounting or legal advice and no attendee should act or refrain from acting on the basis of information in this document without first seeking professional advice.

Locum Agreement

This Locum Agreement Template tool has been customized for physicians practicing in blended capitation groups in Newfoundland and Labrador. It is intended to help you develop a locum agreement. A Locum agreement is an agreement between a hiring/host physician and a locum physician setting out the responsibilities of each of the parties and the financial terms and payment obligations to the locum physician.

How you use this tool will depend on your practice situation.  The template represents one option for developing a locum agreement, but it is not the only way you may choose to structure agreements for having locum physicians work at your practice under Blended Capitation. Calculators

The After-Hours Expectation Calculator is an interactive tool developed by the FPRP’s Blended Capitation Program to assist physicians in estimating their after-hours expectation based on the number of patients rostered to the blended capitation group. By using this calculator, you agree that the returned results are estimates and results may vary from the actual after-hours expectation required under Schedule R.  The calculator uses roster data and physicians are encouraged to review and validate their list of attached patients prior to using the tool for best results.

The BCM Bi-Weekly Estimated Payment Calculator is an interactive tool developed by the FPRP’s Blended Capitation Program to assist physicians in estimating their bi-weekly blended capitation payments based on the characteristics of their patient roster. If you use this calculator, you acknowledge that the returned results are estimates and results may vary from actual payments calculated in accordance with Schedule R. The calculator uses roster data and physicians are encouraged to review and validate their list of attached patients prior to using the tool for best results.

eDOCS NL has prepared a tip sheet that shows how to:

  • run reports to count rostered patients and estimate capitation payments; and
  • obtain a detailed remittance report for fee-for-service claim payments.

Locum Request Form

Any Blended Capitation Physician seeking a locum can use this form to input their requests. Opportunities will be sent out to the active locum group on a weekly basis or as needed if urgent.
