Group Governance Agreement Checklist and Template

These tools have been customized for physicians practicing in blended capitation groups in Newfoundland and Labrador. They are intended to help you develop a group governance agreement. A group governance agreement is a formal written document that addresses the general operations of a group practice and is strongly recommended to document group intentions, and to mitigate potential problems in the future.

How you use the tools will depend on your practice situation. The FPRP recommends meeting with your group to discuss the topics outlined in the Group Governance Checklist to set your intentions as a group before seeking legal or accounting advice, and then to leverage the Group Governance Agreement Template as you formalize your arrangements. The template represents one option for cost-sharing in a group practice, but it is not the only way you may choose to arrange your group practice. It is not meant to be prescriptive: some groups may choose to use the template directly, while others may want to use it as a starting point for customization. Or, if a group already has a group agreement in place, the template may be used to help inform adjustments to address new aspects of group practice that come with blended capitation. This Locum Agreement Template tool has been customized for physicians practicing in blended capitation groups in Newfoundland and Labrador. It is intended to help you develop a locum agreement. A Locum agreement is an agreement between a hiring/host physician and a locum physician setting out the responsibilities of each of the parties and the financial terms and payment obligations to the locum physician.

How you use this tool will depend on your practice situation.  The template represents one option for developing a locum agreement, but it is not the only way you may choose to structure agreements for having locum physicians work at your practice under Blended Capitation.
