Honoraria Claim Forms

Use this form to claim honoraria for work under the Family Practice Renewal Program (FPRP).

Expense Claim Forms

  • Electronic Submission Submitting FPRP-related expenses electronically must be done via Expensify, a free service that automates and manages the process through a mobile app and website.
  • Postal Mail/Fax Submission Expenses claimed using the below PDF forms must be submitted via postal mail or fax. Expense claim forms and associated receipts are no longer accepted via e-mail attachment. Note, processing of these submissions takes longer than those submitted electronically above.
    • Expense Claim Form (PDF) Use this form to claim expenses that are incurred by meeting attendance or travel on behalf of the FPRP. If claiming expenses for more than one purpose (e.g., multiple trips), separate forms must be used.

Direct Deposit Form

Use this form to start, change, or cancel direct deposit. Direct deposit is the preferred method of payment; alternate payment methods (i.e., cheque) will delay payment.
